Emergency Contact Information Collection and Use
We collect information from you about you, your medical information, and your emergency contact person using our In Case of Emergency (ICE) Contact Form.
The only people who will have access to this information is our Webmaster, our Parish Administrator, and our Rector. In the event of an emergency, medical or otherwise, only we or our Vestry, Staff, or other volunteers will use it to assist in the care and medical well being of the people at St John's hosted events.
We guarantee that we will keep your personal information private. We will store your emergency information securely on our server and only access it in the event of a medical emergency. We will not send any email, calls, texts, or any other communication to the provided emergency contact person unless it is an emergency. [Emergency is defined as: a serious, unexpected, and often dangerous situation requiring immediate action or a person with a medical condition requiring immediate treatment.]
By submitting information via this form, you have authorized us to give your medical information to anyone providing medical care including but not limited to CPR & First Aid Providers, Paramedics, or any other emergency care providers.
Information Collection and Use
We collect information from you when you use our web-based tools when you subscribe to follow posts and comments, or fill out a form on our website or any other social media platforms.
When communicating with us through our websites, software, or mobile applications for these purposes you may be asked to enter your name, email address, mailing address, phone number.
Any of the information we collect from you may be used in one of the following ways:
To improve our website based on the information and feedback we receive from you
To respond to your customer service requests and support needs
To evaluate and respond to your guest post submission
You acknowledge and agree that any information you provide to us through our websites, software, or mobile applications is provided voluntarily.
